Create a template

EEE+ Evaluations provides instructors with the capability to create custom evaluation templates. A template has a unique name and a set of customized questions.

Create a template

To create a new template:

  1. Open EEE+ Evaluations If you have not already done so, please login
  2. Click Templates
  3. Click Create a new template
  4. Enter a title for the form and click Create
  5. You will then be presented with a form in which you can add questions or elements

The template will be automatically saved as questions are added or changed.

Duplicate a template

To create a duplicate of an existing template:

  1. Open EEE+ Evaluations If you have not already done so, please login
  2. Click Templates
  3. Click Duplicate in the row that contains the appropriate template

The page will refresh, and your Template Library will contain the new copy.

Use the Midterm Feedback form

When in the Template Library, you have the option to use the the Midterm Feedback Form (DTEI) provided by the Division of Teaching Excellence and Innovation (DTEI). This template cannot be edited, but you can duplicate the form and edit the copy of the form in your Template Library.

To preview the Midterm Feedback Form:

  1. Open EEE+ Evaluations If you have not already done so, please login
  2. Click Templates
  3. Locate the Midterm Feedback Form (DTEI)
  4. Click on Preview to view the form

You can click Done to return to your Template Library.

Edit a template

Once an evaluation is published, the title and settings can be modified, but the questions cannot. Any changes made to questions in the template will not be reflected in the published evaluation.

To edit an existing template prior to publication:

  1. Open EEE+ Evaluations If you have not already done so, please login
  2. Click Templates
  3. Click Edit in the row that contains the appropriate template
  4. Make changes as needed

The template will be automatically saved as questions are added or changed. Click Done when you are ready to leave the template editor.

Create a question

You can begin the process of creating a question by clicking the dropdown at either the top or bottom of the form titled Add question or element. You can then select the type of item you would like to add.

Reorder questions

Reorder Question You can reorder a question by clicking the directional icon, then dragging the question to a new location within the evaluation template.

Reorder Question from the editor Alternately, if you are in the process of editing a question, you can change the ordering using the arrows in the lower right corner of the editor.

Edit a question

Edit Question You can click the pencil icon to begin the process of editing a question within the evaluation template.

Remove a question

Delete Question You can click the trash icon to remove a question from the evaluation template.

Duplicate a question

Duplicate Question You can duplicate a question by clicking the copy icon next to it.

Text Response

There are four types of text response questions:

To create a text question:

  1. Open EEE+ Evaluations If you have not already logged in, please do so
  2. Click Templates
  3. Create a new or edit an existing template
  4. Click Add question or element
  5. Select Text response
  6. Enter the question text
  7. Select the appropriate response option
  8. Click Save

Multiple Choice/Select

There are two types of multiple option questions:

To create a multiple option question:

  1. Open EEE+ Evaluations If you have not already done so, please login
  2. Click Templates
  3. Create a new or edit an existing template
  4. Click Add question or element
  5. Select Multiple choice/select
  6. Type the question text into the question
  7. Configure the question options to identify how many options participants can select
  8. Enter answer options
  9. Click Save question

Multiple choice option remove icon To remove answer options, click the trash can icon.

Multiple choice option reorder icon To reorder the options, you can use the icon to the left of the option to click and drag an option.

Multiple choice option move down icon To move an option down in the list, click the down arrow to the right of the option.

Multiple choice option move up icon To move an option up in the list, click the up arrow to the right of the option.

Contextual Information

You can use a contextualization element to provide the information within the evaluation. This information could include specific instructions, additional information, or other material that the participants would need to complete the question or set of questions.

Note that you can add formatting to the additional contextual information using markdown. For more information about the syntax available in markdown, including bold, italic and links, please click Learn more about Markdown near the lower right corner of the text entry box.
